A retrospective institutional study of the association of smoking with the severity of hidradenitis suppurativa

Highlights

  • Smoking is a modifiable lifestyle factor and a contributing risk factor for HS.

  • In our study of 133 patients, current smoking was associated with more extensive HS.

  • Smokers had increased risk for more than two body areas affected compared to non-smoking patients with HS.

  • Patients should be informed about the effects of smoking in HS and encouraged to quit smoking.

    Reported prevalence rates of HS vary from 0.03% to 4% of the population, with female predominance (Jemec et al., 1996; Kirsten et al., 2020; Revuz et al., 2008; Vazquez et al., 2013). Obesity, smoking, and mechanical friction are well-established risk factors associated with the development of HS (Dessinioti et al., 2017; Fabbrocini et al., 2016). It has gradually been accepted that HS is an immunologic disease of the hair follicle, immunohistologically characterized by perifollicular inflammation predominantly involving neutrophils and macrophages in the acute phase of the lesion.
