Cutaneous malignant melanoma in association with mycosis fungoides

Abstract

We retrospectively analyzed the first 461 cases entered into our cutaneous lymphoma database and found 285 cases of mycosis fungoides. We also identified 6 cases of malignant melanoma, all of which were found in patients with mycosis fungoides. The crude rate of melanoma in the general population in England, United Kingdom, in 1998 was 8.8/100000 in men and 11.4/100000 in women. The incidence of melanoma found in our cohort of patients with mycosis fungoides was far higher, and in 4 of the 6 patients cannot be explained on the basis of prior therapy. The reason for this association is unclear, but this report emphasizes the risk of second malignancies for patients with cutaneous T-cell lymphoma and melanoma.

Case reports

We describe 6 patients (4 men, 2 women) with cutaneous malignant melanoma (MM) in association with MF. The clinical details are summarized in Table I. The mean age of onset of MF was 46.5 years (range: 26-69 years). At diagnosis 2 patients had stage IA, 2 patients had IB, and 2 patients had IIA disease. No patient has developed more advanced lymphoma (median follow-up: 9 years).

Discussion

Retrospective analysis of the first 461 cases entered into our cutaneous lymphoma database revealed 285 cases of MF. Among these cases of MF were 6 patients who also had a MM. In these 6 patients the cutaneous lymphoma was diagnosed first in 4 cases and the melanoma first in 2 cases. During the period of follow-up, 2 patients developed metastatic melanoma but none had progression of their MF.
